Payment Options
For cosmetic procedures, full payment is required two weeks before the procedure. At each patient's initial consultation, the practice's clinical coordinator Carolyn will personally review pricing and payment options with you. Payment options include credit card, check, cash or CareCredit.
Paying with CareCredit
Dr. Cooper offers an easy, credit based payment option through CareCredit. CareCredit is a flexibly patient/client payment program, specifically designed to make it easier for you to get the treatment or procedures you want. CareCredit lets you begin your treatment or procedure immediately — then pay for it over time with low payments that are easy to fit into your monthly budget. CareCredit offers a full range of No Interest and Extended Payment Plans for treatment fees up to $25,000.

Insurance, Referrals & Authorizations for Covered Procedures
Dr. Cooper's goal is to provide his patients with timely access to highly specialized plastic and reconstructive surgical care. If you are interested in reconstructive surgical services, please remember that at times, referrals to specialty health services require prior authorization. Our team will work collaboratively with your referring physician's office and your insurance plan to ensure that all of the requirements for referral authorization are met.
We encourage you to review your coverage policy prior to your visit with us. Each plan is different and it is important to understand the extent of your responsibilities, if you have any co-payment or deductible requirements, and if there are any limitations on how you can access the specialty care services you may sometimes need. If you have any questions, please contact your health plan's member service department.
Hospitalization
Patients in need of immediate hospitalization or inpatient services will be admitted to Northwest Hospital & Medical Center unless requested otherwise.
